Probably a good way to answer these questions is to take a look at the obligatory refund reasons within Refund Policy for Authors. Note, these are the kinds of situations in which we expect an author to provide a refund because legally, you're required to.
1. Item is "not as described"
2. Item doesn't work the way it should
3. Item has a security vulnerability
4. Item support is promised but not provided
5. Item support extension not used
6. Items that have not been downloaded
You'll note that all of these obligatory refund reasons are not things that we can control within our review process. Review exists to examine the integrity and quality of the item itself, not how an author chooses to describe their item, whether or not that's accurate, or an author's ability to deliver upon their support obligations.
These are things we trust our authors to manage themselves.
Absolutely. Author replies to comments will also be visible. This will allow you to both transparently support the customer if they are in need to support, and convey the circumstances to other buyers.
